Awana BC Camp - Camper Application

Congratulations on qualifying for Awana BC Camp.

- Medical Information: Doctor's contact, Care Card/Medical Insurance Number, Medication, Date of last Tetanus shot, Allergies
- Year Awana awards were earned
- Last two Awana books completed

    Because we still require a physical signature, you will need to download and fill out the Camper Verification Form. Please sign and submit this by mail to the address listed on the form.

    Your Commander or Director will also need to complete a portion of the Camper Verification Form. They may complete this form as soon as you finish your Awana material.

    Please note: the application is not complete until Camper Verification Form and Registration Fee are submitted.
